How Jay Care handles information across the website, intake process and care operations.

Last updated: September 17, 2026

Jay Care Mental Health Center ("Jay Care," "we," "us," or "our") respects your privacy. This Privacy Policy explains how we collect, use, disclose and protect information when you visit our website, contact our office, complete an intake form, request an appointment or use our patient and staff portals.

Important: This website is not an emergency service. If you or someone you know is struggling or in crisis, call or text 988 or chat at 988lifeline.org. If there is immediate danger or a medical emergency, call 911 or go to the nearest emergency room.

1. Information we collect

Depending on how you interact with us, we may collect contact information, identity and demographic information, appointment preferences, insurance and billing information, information you provide about your health and care needs, emergency-contact details, and information needed to operate staff accounts and patient records.

We may also collect device, browser, IP-address and security-log information needed to protect the Services. We do not currently use optional analytics or advertising trackers.

2. How we use information

  • To respond to questions, schedule and coordinate care, and provide requested services.
  • To review and process patient intake information.
  • To communicate about appointments, forms, care coordination, billing and administrative matters.
  • To maintain patient, staff and operational records.
  • To protect the website, prevent misuse and meet legal, regulatory and professional obligations.
  • To improve the website, workflows and patient experience.

3. Protected health information

When Jay Care creates, receives, maintains or transmits protected health information in connection with healthcare services, that information is handled under applicable federal and state privacy requirements, including HIPAA where applicable. Our separate Notice of Privacy Practices describes permitted uses and disclosures of protected health information and your rights. A website privacy policy does not replace that notice.

4. How we share information

We may share information with clinicians, care teams, service providers, technology vendors, billing and insurance partners, appointment and health-record systems, and professional advisers when needed to operate the practice or provide care. We may disclose information when required by law, to protect safety, or with your authorization. We do not sell personal information.

5. Forms, portals and manual record transfer

Online forms and portals may be hosted or supported by Supabase and the deployment provider selected by Jay Care. Authorized office staff may download a structured packet and manually copy information into classic iCanotes or another system selected by Jay Care. This website does not connect to, sync with or send data automatically to iCanotes. Access to patient information and downloaded packets must be limited by role and operational need.

6. Cookies and analytics

We use strictly necessary cookies and similar technologies for security, session management and preferences. We do not currently use optional analytics or advertising trackers.

7. Your choices and rights

You may contact us to ask questions about this policy or to request access, correction or other rights available under applicable law. Healthcare-record requests may be subject to procedures and limits described in our Notice of Privacy Practices.

8. Security and retention

We use administrative, technical and physical safeguards appropriate to the information we handle. No internet transmission or storage system can be guaranteed completely secure. We retain information for as long as reasonably necessary for care, business, legal, accounting, insurance and recordkeeping purposes.

9. Children

Our services may involve minors when accepted by the practice and appropriate consent is obtained. We do not knowingly collect information from children through general website features without appropriate authorization.

10. Changes

We may update this policy as our services, technology or legal obligations change. The updated version will be posted on this page with a revised date.
