Terms of Service
The terms for using the Jay Care website, forms and team workspace.
Last updated: September 17, 2026
These Terms of Service ("Terms") govern your use of the Jay Care Mental Health Center website, online forms, appointment-request tools and related patient or staff portals (collectively, the "Services"). By using the Services, you agree to these Terms. If you do not agree, do not use the Services.
1. Healthcare relationship
Website content is for general information and does not provide medical advice, diagnosis or treatment. Completing an online form, sending a message or requesting an appointment does not create a provider-patient relationship or guarantee an appointment. A healthcare relationship begins only when accepted and established through Jay Care’s clinical process.
2. Appointments and communications
Appointment requests are not confirmed until Jay Care confirms them. We may contact you using the information you provide about scheduling, forms, care coordination or administrative matters. You are responsible for providing accurate information and updating it when it changes.
3. Patient information and privacy
Information submitted through the Services is handled as described in our Privacy Policy and, where applicable, Jay Care’s Notice of Privacy Practices. Do not use public website fields to submit information about someone else unless you are authorized to do so.
4. Accounts and staff access
Staff-portal access is limited to authorized Jay Care personnel. Users must keep credentials confidential, use only the records and functions authorized for their role, and report suspected unauthorized access promptly. Staff must not download, copy, disclose or use patient information outside approved workflows.
5. Acceptable use
You agree not to interfere with the Services, probe or bypass security controls, impersonate another person, upload malicious code, use the Services to harass or threaten others, or submit unlawful, fraudulent or intentionally misleading information.
6. Third-party services and links
The Services may link to or integrate with third-party services such as telehealth, appointment, payment, electronic-health-record or crisis-support tools. Jay Care is not responsible for third-party content, availability, privacy practices or terms. Review the applicable third-party materials before using those services.
7. Intellectual property
Unless otherwise stated, the design, text, branding and original materials in the Services belong to Jay Care or its licensors. You may use the Services for personal, noncommercial purposes related to seeking or receiving care. You may not reproduce, modify, distribute or exploit site materials without permission.
8. Disclaimers
The Services are provided on an “as available” basis. We do not guarantee that the website or forms will always be available, error-free, secure or suitable for every purpose. Information on the website may change and should not be relied on as a substitute for professional medical advice.
9. Limitation of liability
To the extent permitted by law, Jay Care and its personnel will not be liable for indirect, incidental, special, consequential or punitive damages arising from use of the Services. Nothing in these Terms limits rights or obligations that cannot be limited under applicable law.
10. Indemnification
To the extent permitted by law, you agree to defend and hold harmless Jay Care from claims arising from your misuse of the Services, violation of these Terms or violation of another person’s rights.
11. Governing law
These Terms are governed by applicable federal law and the laws of the State of Maryland, without regard to conflict-of-law principles, unless applicable law requires otherwise.
12. Changes
We may update these Terms as the Services or legal requirements change. Continued use after an update means you accept the revised Terms.
